Type 3 gynecomastia has a chest angle of 45-60° with only mild breast sag due to the angle involved. In contrast, type 4 is characterized by a chest angle 60-90° with moderate and noticeable chest sag. Type 5 has a chest angle of greater than 90°, with resultant significant sag.
